Errors in Radiology - A Holistic Approach

FOCUS: In common with any human activity, errors are an unfortunate fact of life for all radiologists from recruitment to retirement. We cannot eradicate error, but we can examine the underlying causes, and devise mitigating processes. When an error leads to patient harm, there is a duty of candour, possible legal redress and maybe also a coroner’s inquest. How does the Reporter deal with such circumstances?
What does the Royal College of Radiologists do to educate regarding error? Can we, and should we identify an error prone personality during the recruitment process? Can we identify and mitigate error prone technique post recruitment? Can AI help?
AIM: This seminar aims to examine all aspects of this important issue, and promote an in depth, team-based approach to the reduction of radiological error.
TARGET AUDIENCE: Any NHS imaging professional working within the North West Region who reports on images.
